Risk Adjustment Medical Coder

Chart Review: Systematically review outpatient and inpatient medical records to capture all active chronic conditions (e.g., Diabetes, COPD, CHF).
HCC Coding: Assign accurate ICD-10-CM codes that map to HCC categories according to CMS (Centers for Medicare & Medicaid Services) guidelines.
Documentation Gap Analysis: Identify clinical "clues" in the documentation (like medications or lab results) that suggest a condition exists but hasn't been explicitly coded, and flag these for physician query.
Audit Support: Participate in Risk Adjustment Data Validation (RADV) audits to defend the validity of submitted codes.
Quality & Productivity: Maintain high accuracy (typically 95-98%) while meeting daily chart-per-day targets.
