The Community Care & Social Services Coordinator is responsible for coordinating community-based healthcare and social support services to ensure individuals and families have access to appropriate care and available resources. The role collaborates with healthcare providers, social workers, government agencies, community organizations, and other stakeholders to assess needs, coordinate referrals, facilitate access to health and social services, and support care planning. The Coordinator monitors service delivery, maintains accurate records, addresses client concerns, and promotes programs that improve health, well-being, and quality of life while ensuring compliance with organizational policies and applicable regulations.
