Patient Care Coordinator (Remote)

Responsibilities:
Check in with patients regularly by phone
Remind patients to take their prescribed medications
Follow up to make sure patients are keeping up with their care plans
Help coordinate appointments and follow-ups
Keep accurate records of patient communications
Report any patient concerns to the appropriate medical staff
Requirements:
Excellent spoken and written English
Previous healthcare or patient coordination experience preferred
Friendly, patient and comfortable speaking with people
Organized, reliable and able to work independently
Comfortable making outbound calls
