We are seeking a compassionate and dedicated Community Registered Social Worker to join our organization in the Civic & Social Organization industry. As a Community Social Worker, you will be responsible for providing vital support and resources to individuals and families facing various challenges. Your work will be instrumental in improving the well-being of our community members and empowering them to lead fulfilling lives.
Description
Registered Social Worker
- Conduct comprehensive assessments to identify the needs and challenges of individuals and families in the community.
- Develop and implement personalized care plans in collaboration with clients, helping them set achievable goals and overseeing their progress.
- Provide counseling, emotional support, and crisis intervention to individuals and families in need.
- Connect clients to available community resources, such as healthcare services, housing options, employment assistance, and financial support programs.
- Advocate for clients' rights and coordinate with other professionals, agencies, and organizations to ensure they receive appropriate services and support.
- Facilitate support groups and educational workshops to enhance clients' social and life skills.
- Conduct home visits and follow-up appointments to monitor clients' well-being and address emerging needs.
Job Requirements
· Bachelor's in Social Work, Registered Social Worker
· Strong understanding of local social service systems and resources, and knowledge of relevant legislation and regulations.
· Excellent communication skills, both verbal and written, with the ability to work effectively with diverse populations.
· Empathy, compassion, and strong interpersonal skills to build rapport and establish trusting relationships with clients.
· Strong organizational and time-management skills, with the ability to prioritize tasks and meet deadlines.
· Proficient in using computer applications and relevant software for documentation and reporting.
· Preferably a resident of SJDMC, Bulacan, Caloocan or nearby areas.
