Care Coordinator (Registered Nurse & Licensed Practical Nurse)

Job Summary:

The Care Coordinator is responsible for providing patient-centered care through telephonic and virtual support. This role focuses on patient assessment, care planning, coordination of services, and ongoing monitoring to ensure optimal health outcomes and continuity of care.

Key Responsibilities:

• Conduct telephonic and virtual patient assessments and provide ongoing monitoring
• Develop, implement, and update individualized care plans based on clinical guidelines
• Educate patients and families on conditions, medications, and self-management strategies
• Monitor patient progress and escalate clinical concerns when necessary
• Coordinate care across healthcare providers, payers, and community resources
• Ensure accurate and timely documentation in EHR and care management systems
• Collaborate with clinical and operational teams to improve patient engagement and outcomes

Qualifications:

• 6 months to 2 years of direct clinical experience (acute care, ambulatory, home health, or care management)
• Strong clinical assessment and critical thinking skills
• Knowledge in chronic disease management, preventive care, and health coaching
• Ability to interpret clinical data, medical instructions, and test results
• Excellent communication skills (verbal and written)
• Comfortable working in a collaborative, fast-paced environment
