Overview

We are seeking an experienced Home Health QA /QAPI - Registered Nurse to support clinical quality assurance, Quality Assessment and Performance Improvement (QAPI), documentation review, and daily clinical operations.

Initially, this position will focus on addressing an existing QAPI backlog, organizing quality reporting, conducting targeted chart reviews, and establishing consistent clinical tracking and reporting processes. The role will work closely with the Director of Nursing (DON) to identify documentation deficiencies, monitor clinical quality indicators, and escalate issues requiring licensed clinical review.

The ideal candidate has extensive U.S. home health experience, strong knowledge of Medicare documentation requirements, proficiency with WellSky/Kinnser, and the ability to independently manage multiple quality and operational priorities.

Clinical QA & Documentation Review

• Review Start of Care (SOC), Resumption of Care (ROC), Recertification, Discharge, and other clinical documentation for completeness, consistency, and compliance.

• Review admission records and referral documentation to summarize diagnoses, skilled needs, ordered disciplines, and important clinical conditions.

• Identify missing or inconsistent documentation involving wounds, catheters, IV therapy, medications, physician orders, and plans of care.

• Review Face-to-Face (F2F) documentation for support of home health services and ordered disciplines; flag deficiencies for DON/intake follow-up.

• Perform targeted chart audits involving skilled need, homebound status, medication changes, physician notifications, visit frequency, and teaching progression.

• Monitor outstanding documentation and coordinate corrections with appropriate clinical staff.

• Support existing external QA processes through supplemental chart audits and documentation tracking.

SOC Handoffs & Visit Coordination

• Prepare concise SOC handoff summaries using WellSky/Kinnser yellow sticky notes or KMail for the next visiting nurse.

• Summarize diagnoses, hospitalization history, nursing priorities, wound/catheter/IV needs, safety concerns, and required follow-up.

• Prepare visit-by-visit focus-of-care notes based on established physician orders and plans of care.

• Suggest documentation-supported teaching topics, including medication education, fall prevention, disease management, caregiver education, and discharge readiness.

• Identify new clinical issues that may require orders or plan-of-care updates and escalate findings to the DON.

• Maintain admission, ROC, recertification, and discharge tracking, including upcoming clinical deadlines.

Clinical Surveillance & Quality Tracking

• Maintain active wound surveillance records and review wound notes, measurements, drainage descriptions, and available images

• Track wound progression and prepare weekly wound summaries identifying concerns requiring DON review.

• Maintain fall and incident logs, including circumstances, injuries, notifications, hospital transfers, and follow-up.

• Maintain infection surveillance records tracking infection types, treatments, recurrence, hospitalizations, and documented outcomes.

• Track hospitalizations and emergency room visits, categorizing causes and identifying recurring clinical patterns.

• Identify trends, missing documentation, and potential quality concerns for leadership review.

QAPI Reporting & Performance Improvement

• Organize and address the agency's existing QAPI backlog.

• Maintain monthly quality dashboards tracking admissions, recertifications, discharges, hospitalizations, ER visits, falls, infections, wound outcomes, complaints, and documentation findings.

• Analyze monthly trends and prepare concise quality summaries for leadership.

• Compile quarterly QAPI reports covering performance trends, deficiencies, corrective actions, and measurable outcomes.

• Support performance improvement projects by tracking interventions, responsible parties, progress, and results.

• Prepare reports for DON, Administrator, and Governing Body review and approval.

DON Reporting, Compliance & Survey Readiness

• Prepare a consolidated Monday clinical and compliance report for the DON.

• Include census movement, upcoming recertifications, wound status, falls, infections, hospital activity, F2F deficiencies, missing orders, and outstanding documentation.

• Maintain a prioritized "DON Action Required" section identifying issues requiring clinical decisions or follow-up.

• Maintain organized CHAP/survey-readiness files, including QAPI reports, meeting minutes, audits, incident logs, corrective actions, and improvement documentation.

• Support Medicare home health compliance and ongoing audit readiness.

Additional Operational Support

• Assist with intake, referral processing, scheduling coordination, and outstanding clinical/administrative tasks when needed.

• Monitor incomplete or aged tasks within WellSky/Kinnser and coordinate follow-up.

• Assist with physician order tracking and communication between clinical and administrative teams.

• When assigned, prepare and track DocuSign admission consent packets, including delivery, completion monitoring, and filing.

• Support additional clinical quality and operational projects as agency priorities evolve.

Required Qualifications

• Registered Nurse (RN) or equivalent nursing qualification.

• Minimum 3 years of experience supporting U.S.-based home health agencies in clinical QA, documentation review, or related operations.

• Demonstrated experience reviewing home health clinical documentation and identifying compliance deficiencies.

• Strong understanding of Medicare home health requirements, SOC, ROC, Recertification, F2F, Plans of Care, and physician orders.

• Experience with QAPI reporting, quality tracking, or clinical performance improvement.

• Strong proficiency with WellSky/Kinnser preferred.

• Excellent written and verbal English communication skills.

• Exceptional organization, attention to detail, independent follow-through, and ability to prioritize competing deadlines.

• Comfortable communicating with clinical leadership and supporting multiple operational functions.

Preferred Qualifications

• Experience supporting a Texas-based Medicare-certified home health agency.

• Familiarity with Palmetto GBA requirements and CHAP survey readiness.

• Experience maintaining wound, infection, fall, and hospitalization surveillance reports.

• Previous responsibility for monthly/quarterly QAPI reporting and performance improvement projects.

• Experience with home health intake, scheduling, DocuSign, and clinical operations support.
