Glory Reborn Organization, Inc. is looking for a compassionate and mission-driven Maternal Social Welfare Liaison to support mothers and families by coordinating access to social welfare services and government assistance programs. This role serves as a bridge between clients, government agencies, and partner institutions to ensure timely referrals, proper case documentation, and holistic support for families in need.
Key Responsibilities:
- Coordinate referrals and assistance with DSWD, LGUs, and partner agencies
- Conduct patient intake interviews and maintain case documentation
- Monitor maternal social welfare cases and referral follow-through
- Prepare reports and ensure confidentiality of client records
Qualifications:
- Graduate of Social Work or any related field
- Experience in social services, case management, or government coordination is an advantage
- Strong communication, documentation, and interpersonal skills
- Compassionate, organized, and committed to serving mothers and families
