Job Title: Licensed Practical Nurse (LPN) Care Coordinator

Skills: 6 Months to 2 Years of direct clinical experience required (at least one of the following areas: acute care, ambulatory care, home health, or care management)

Experience: Clinically competent handling EMR, triage patient needs, coordinate care.

We at Coforge are hiring Licensed Practical Nurse (LPN) Care Coordinator - with the following skillset:

• Coordinating with different departments, including physicians, pharmacy, laboratory, radiology, and etc.
• Ability to interpret clinical data, medical instructions, and test results.
• Handled vital signs, patient data, and direct care in hospital and bedside settings.

Responsibilities:

• Document all interactions accurately and promptly in electronic health records (EHR) and care management platforms.
• Use sound clinical judgment to triage patient needs, coordinate care, and escalate appropriately.
• Strong clinical assessment skills with understanding of chronic disease management, preventive care, and health coaching.
• Provide telephonic and virtual patient assessments, care coordination, and ongoing monitoring for assigned patient populations.
