The Medical Social Worker i s responsible to provide professional care to those who are in especially difficult circumstance, responsive to the psycho-social needs of the vulnerable patient and their families.
DUTIES AND RESPONSIBILITIES:
1. Conducts psychosocial assessment and classify patients according to their economic status and other health needs.
2. Provides psycho-social casework to prevent social breakdown by restoring social functioning of the patient.
3. Provides counseling to patients in need.
4. Conducts a multi-disciplinary team consultation and attends case conferences.
5. Establish linkages to funding agencies, and provides referrals and networking.
6. Maintains records and documentation of social work services.
